What is an apicoectomy?

In an apicoectomy, the tip of the root (the root apex) is treated through a minor surgical procedure, along with the inflamed tissue around it. The goal is to remove the inflammation at the root tip and, if possible, preserve the tooth.

When is an apicoectomy considered?

For example, if:

  • there are complaints or inflammation at the root tip that persist after a (re)treatment of the root canal
  • a retreatment is not feasible, for instance because a crown or post cannot be safely removed
  • the root tip is shaped in such a way that the canal cannot be properly cleaned

It is not automatically the next step, but a choice after examination.

Preliminary examination

The dentist examines the tooth, the root tip, and the surrounding bone, using X-rays and, if necessary, a CBCT scan (3D X-ray). This makes it clear how the root is positioned, how large the infection is, and how close important structures, such as nerves or the sinus, are located.

How does the treatment proceed?

  • Anesthesia: the treatment is carried out under local anesthesia.
  • Access to the root tip: through a small incision in the gum.
  • Removal of the root tip and the infected tissue.
  • Sealing the root canal: at the tip of the root, with a special filling material.
  • Suturing: the gum is repositioned and sutured.

After the treatment, you may experience swelling, sensitivity, and some pain. You will receive clear instructions on eating, cleaning, and painkillers. The sutures are removed after about a week, or they dissolve on their own.

Referral to a specialist

Depending on the situation, we either carry out the treatment ourselves or refer you to a specialist. We discuss that clearly with you in advance.

Alternatives

  • Retreatment of the root canal (re-treatment)
  • Extraction of the tooth and replacement, for example, with an implant
  • Monitoring without immediate treatment if you have no symptoms

Does an apicoectomy hurt?

The procedure is performed under local anesthesia. Afterwards, you may experience pain and swelling, which are usually manageable.

How long does the treatment take?

That depends on the tooth and situation. The dentist will provide an estimate beforehand.

How long does recovery take?

The first few days are the most sensitive. The gums usually heal in about a week, while the bone around the root tip requires more time. During check-ups, we monitor whether the healing is progressing well.

Does 3D Tandarts perform this treatment in-house?

That depends on the situation. Sometimes we perform the treatment ourselves, sometimes we refer you to a specialist.
